The aims of this study is Proteus mirabilis as a major cause of complicated urinary tract infections with rising antibiotic resistance. Clinical isolates were identified and confirmed to carry the zapA and ureR genes, alongside high resistance rates to several antibiotics. Lipopolysaccharides (LPS) were extracted and purified, showing inhibitory effects on biofilm formation and hemolysin production. Gene expression analysis by qRT-PCR revealed a concentration-dependent effect, where low LPS levels upregulated virulence genes, while higher concentrations suppressed them. These findings suggest that LPS can act as a signaling molecule regulating pathogenicity, offering a promising avenue for alternative therapeutic strategies beyond conventional antibiotics. The main objective of this study is to investigate the regulatory role of LPS from multidrug-resistant Proteus mirabilis on virulence gene expression and pathogenic behaviors, as a step toward alternative therapeutic strategies.
